Commonly affects medium sized arteries.1 Although this patient did not have Buerger's disease, modestly increased levels of serum homocysteine have been reported in patients with Buerger's disease, 3 and it has been suggested that this may be a contributory factor in the pathogenesis. The enzyme MTHFR is required for remethylation of homocysteine to methionine. The most common alteration in the MTHFR gene is a point mutation C677T ; that causes an alanine-to-valine substitution.4 The allele frequency of the MTHFR C677T mutation is 35%, with a homozygous rate of 12%.4 Homozygosity is associated with modest elevations in plasma homocysteine but low levels of folate or vitamin B12 lead to a relatively large increase.5 Under these circumstances simple treatment with folic acid and vitamin B12 supplementation may be extremely effective in reducing serum homocysteine. As well as reducing the serum homocysteine, folic acid and B vitamins may prove protective in hyperhomocysteinaemia induced atherosclerosis, and lowering homocysteine levels has been shown to reduce restenosis rates after coronary angioplasty.6 Vitamin B6 is a cofactor for the transsulphuration of homocysteine metabolism fig 1 ; , and depleted vitamin B6 levels may also contribute to raised homocysteine. We believe that this is a case of hyperhomocysteinaemia, initially misdiagnosed as Buerger's disease. We suggest that if Buerger's disease, a diagnosis of exclusion, is to be sustained then serum homocysteine should be added to the list of screening investigations.1 This is of particular importance since, as this case demonstrates, simple vitamin and folic acid supplementation can normalise homocysteine levels and aprepitant.
Animals. Studies were carried out in male Wistar rats weighing between 200 and 300 g, bred in the animal facilities of the Faculty of Medicine, Universidad Nacional Autonoma de Mexico. They were subjected to a 12-h light dark cycle and were maintained at 21 23C; tap water and food pellets 5001 Rodent Laboratory Chow; Agribrands Purina Canada, Woodstock, ON, Canada ; were available freely. Animals were brought daily to the laboratory for the experiments, which were conducted according to the Guide for the Care and Use of Laboratory Animals National Institutes of Health Publication 8523, revised 1996 ; . Experiments in Anesthetized Rats. Animals were anesthetized with a mixture of chloralose, 50 mg kg, and urethane, 800 mg kg, administered i.p. Cannulas were placed in the trachea to facilitate spontaneous breathing, in a femoral artery for blood pressure recording, and in a femoral vein for drug administration. The arterial cannula was filled with heparin, 50 U ml, to prevent clotting; no anticoagulant was administered systemically. Variations in PROP PTC sensitivity appear to arise from underlying anatomical differences, since ratings of PROP intensity are highly correlated with the density of fungiform papillae on the tongue Miller and Reedy, 1990; Bartoshuk et al., 1994 ; . This being the case, it is not surprising that the degree of PROP PTC sensitivity has also been associated with variations in the perception of a number of other taste compounds. If there are differences between individuals in their sensitivity to individual tastes, the question arises of how this translates into the experience of taste qualities during the consumption of foods and beverages. Clearly, the most common experience of tastes is in the form of mixtures of two or more different taste qualities such as sweetness and sourness or sweetness and bitterness--both combinations being characteristic of fruit juices, for example.
