The role of information and communication technology in the development of the Pacific island countries, particularly in meeting the Millennium Development Goals. These programmes were aimed to raise awareness about the Millennium Development Goals and explore whether information and communication technology could be an effective mechanism to achieve them; and to encourage an informed debate amongst government and non-government stakeholders, including members of national Information and Communication Technology Councils and relevant NGOs, in strategy development and implementation. The programmes were broadcast in all three countries concerned and have been made available to other broadcasters in the Pacific island region. UNESCO's contribution to this project was earmarked for the capacity building of the local production teams, as well as to contribute to locally relevant content in the small islands electronic media. Getting Our Own Stories on the Screen In collaboration with the Pacific Islands News Association PINA ; , UNESCO sought to promote culturally diverse local content in the Pacific Islands Television, through three activities in 2004. The first one was a seven-day workshop in November, "Getting Our Own Stories on the Screen, Writing for TV Drama". The second one is an IPDC project entitled "Pacific Television Programming for Community Development", which was prepared in 2004 and will start in early 2005. UNESCO also continued its support to the monthly TV magazine "The Pacific Way", which is being produced by the Secretariat of the Pacific Community SPC ; and is a spin-off from an earlier IPDC project. "Getting Our Stories." brought together participants from television stations of Cook Islands, Federal States of Micronesia, Fiji, Niue, Tonga, Samoa and Vanuatu.

Chemoprevention of cancer is reviewed from the viewpoints of action mechanisms and methodology of clinical trials in order to introduce promising agents discovered by in vitro and or in vivo studies to applications in humans. The clinical trial procedure essentially follows the phase study which has been employed for chemotherapeutic drugs. Chemoprevention of bladder cancer, prostate cancer, gastric cancer, hepatocellular carcinoma, breast cancer, head and neck cancer, colorectal cancer and lung cancer is reviewed, mainly focusing on clinical trials. Previous clinical trials have shown the effectiveness of the following: polyprenoic acid acyclic retinoid ; for hepatocellular carcinoma; tamoxifen for breast cancer; retinoic acids for head and neck tumor; and aspirin, a COX-2 inhibitor, for colorectal cancer. Despite the advantageous effects of some of these agents, their toxic effects must also be of concern at the same time. For example, in a chemoprevention trial of lung cancer, b-carotene was unexpectedly found to increase the risk of lung cancer among high-risk groups. It is also noted that large-scale clinical trials demand large research grants, which may not be affordable in Japan. In 5 of the 7 cases sustained engraftment of donor cells was observed. Two patients developed graft failures: in patient UPN 275 graft function deteriorated at 1 month in the context of CMV disease, which was reversible by a second graft on day 42 from the same donor without further conditioning. This complication may reflect the vulnerability of the reconstituting hematopoietic system after purified blood progenitor cell transplantation. In the other patient UPN 292 ; the graft was rejected on day 15, following conditioning with a lowered total dose of cyclophosphamide 120 mg kg instead of 200 mg kg ; . This child died from CMV pneumonia shortly after a second transplant from the opposite parent. Graft rejection in this case may emphasize the critical role of the intensity of conditioning, even if high numbers of stem cells are used for transplantation. It should also be pointed out that in the other patients complete hematologic recovery with development of normal blood cell counts was delayed Table 2 ; . This delay may be related to the occlusion of marrow space characteristic for the disease. In an attempt to shorten the time interval to hematologic recovery, additional donor CD34 cells were infused at around 1 week after HSCT in 5 of the 7 patients. Although no effect on the slow kinetics of hematologic recovery was evident, regardless of the number of CD34 cells infused, this approach may have led to stabilization of engraftment, because none of the patients developed graft failures. Background: The use of conventional foldable hydrophobic intraocular lenses IOLs ; in microincision cataract surgery MICS ; currently requires wound enlargement. We describe a combined surgical technique of MICS and trabeculectomy with insertion of a foldable IOL through the trabeculectomy fistula. Methods: After completion of MICS through two side port incisions, a 3.2 mm keratome is used to enter the anterior chamber under the previously outlined scleral flap. An Acrysof multi piece IOL Alcon labs, Fort Worth, Tx ; is inserted into the capsular bag through this incision. The scleral flap is then elevated and a 2 fistula made with a Kelly's punch. The scleral flap and conjunctival closure is performed as usual. Results: Five patients with primary open angle glaucoma with a visually significant cataract underwent the above mentioned procedure. An IOL was implated in the capsular bag in all cases with no intraperative complications. After surgery, all patients obtained a best corrected visual acuity of 20 IOL was well centered at 4 weeks follow up. The mean IOP without any antiglaucoma medication ; was 13.2 + 2.4 mm Hg at weeks with a well formed diffuse filtering bleb in all the cases.
